Parental declaration form
The declaration is the main agreement for the funded early education entitlement (FEEE) place between the parent and the provider. This form must be completed, alongside parents, for all funded children.

Please note that this form does not replace the usual (full) parental contract that providers will have with each parent.
Providers are not required to submit parental declaration forms to the council but must retain a copy for audit purposes and give a copy to the parent/carer for their reference.
All forms already completed and held by providers will remain valid and do not have to be replaced, unless changes are required.
