Children's continuing care
The National Framework for Children and Young People’s Continuing Care sets out the criteria for access to a package of continuing care.
These packages are for the children and young people with the most complex health conditions that require ongoing management, such as children with a tracheotomy or those who require respiratory ventilation.
If you are eligible for a package, your needs will be assessed and an appropriate package offered to you via our local Diana Nursing Service. The service endeavours to be as flexible as it can be and will tailor your package to your individual needs and the needs of the family. It is a 24/7 service.
If you are eligible for a package of continuing care, you may decide that you do not want to access our local service but would prefer to have your health needs met by way of a personal health budget. Transforming care for children and young people
The Leicester, Leicestershire and Rutland Partnerships is working with families and partners in health and social care to develop good local services that will help people with a learning disability and/or autism to stay well in the community rather than be admitted to in-patient settings.
The Partnerships focus is on identifying individuals who are at risk of being admitted to hospital and working to prevent this whilst also making sure there is the right support for people to be safely discharged from hospital at the right time, and remain in the community where possible.
Care Education and Treatment Reviews (CETRs) improve the care of people with learning disabilities, autism or both with the aim of reducing admissions and unnecessarily lengthy stays in mental health hospitals and reducing health inequalities. The CETR bring together all responsible services to ensuring that the care and treatment of the person and their families are met.
If you feel someone maybe at risk of hospital admission, please talk to your key worker and CAMHS who can discuss the case and make a referral to the Risk of Admission Register (ROAR) which is held by the Clinical Commissioning Group who can then arrange a CTER.
