Transition support for young people consultation
Your view are sought on the transition support for young people.
Until a young person is 18 years old they can receive health and social care support from children’s services. Once someone turns 18 the law says they are an adult. To help make sure that young people with additional needs in Leicester are best supported as they become adults, staff from Leicester City Council and the NHS have worked together to produce a document called the ‘Joint Health, Social Care and Education Transitions Strategy’.
The Council want to make sure that the strategy has all the important details in. They also want to check that they have worked out the best ways to make improvements.
This consultation will be open until 8 February 2019.
